Application
Fill out our online application. There is no evaluation fee. Along with the form, please provide the supporting documentation listed below.
Assessment
Once we receive your application, our team will schedule a full day on campus. Our school director and onsite BCBAs evaluate academic, motor, social, and behavioral skills. Literacy is assessed using the Basic Reading Inventory. You'll be notified within 7–10 business days.
Registration
Once accepted, families have 14 business days from the emailed notification to return the completed registration packet.
Documents to provide
Unlike many schools in our field, we do not charge an evaluation fee. Please share the following with your application:
- Copy of last IEP (Individualized Education Plan)
- Copy of Behavior Intervention Plan, if applicable
- Copy of most recent psychological evaluation
- Copy of last year's report card
- Copy of most recent standardized test score, if applicable
Admissions requirements
Golden Horizon Academy serves students who meet the following criteria:
- Students with mild to high developmental and intellectual disabilities
- Students must demonstrate independence in toileting and eating
- Students entering Kindergarten through Eighth Grade
- Students entering Kindergarten must be five years old by September 1st
